What Exactly Is a Trigger Point and Why Does It Cause Pain?
You've probably experienced one without knowing the name. A specific spot in a muscle — often in the upper trapezius, the rhomboids, or the neck — where pressure produces a distinctly different sensation than surrounding tissue. Sometimes a local sharp pain. Sometimes a dull, deep ache. Often a referred pain pattern — pressure at the spot causes sensation somewhere else entirely, in a predictable location specific to that trigger point's location.
That's a myofascial trigger point: a hyperirritable spot within a skeletal muscle, associated with a hypersensitive palpable nodule in a taut band of muscle tissue. The physiological mechanism involves abnormal electrical activity in a localized area of muscle, resulting in a persistent contraction that doesn't release normally.
Trigger points are extremely common — most people who experience persistent muscular pain have them, often multiple. They develop from acute muscle overload, sustained postures (particularly the forward head and rounded shoulder posture of screen work), repetitive motions, chilling or direct trauma to muscle, and as secondary effects of emotional stress and poor sleep.

The referred pain patterns are consistent enough across individuals that a skilled therapist can often predict which trigger points to look for based on where the client describes their pain.
Ischemic Compression
The primary technique: sustained direct pressure on the trigger point, typically held for 8-12 seconds or until the client reports a release in the pain intensity. The pressure is uncomfortable — often producing the referred pain pattern the client recognizes as their complaint — but not beyond tolerance. The therapist maintains the pressure while the client breathes and consciously releases tension in the area.
After a successful release, the area is worked with broader strokes to restore blood flow and lengthen the previously shortened muscle. The entire process is then repeated for any remaining trigger points in the session.
Muscle Energy Techniques
Some trigger point work incorporates active participation from the client — gentle contractions against resistance, followed by release and stretching, to help the muscle reset to normal resting length after trigger point release.
What Do Clients Experience During Trigger Point Therapy?
Trigger point therapy feels different from standard massage. The experience is more targeted and more intense — you're working on specific spots that are producing real pain, which means the treatment itself produces some discomfort. This is sometimes described as "hurts so good" — an intensity that feels purposeful and productive rather than just painful.

This recognition ("that's my headache!") is often a satisfying moment of understanding a pain pattern that may have been confusing for months or years.
