Is Prenatal Massage Safe?

This is the first question most women (and their families) ask, and it's the right one. The short answer: yes, prenatal massage is safe for most pregnant women, with some important qualifications.

The qualifications: prenatal massage should be avoided in the first trimester (the first 12-13 weeks) except under specific medical guidance. This isn't because massage is dangerous in the first trimester, but because miscarriage risk is highest during this period and massage associations and most therapists recommend avoiding it to err on the side of caution. From the second trimester onward, prenatal massage is generally considered safe for low-risk pregnancies.

If you have any of the following, get clearance from your doctor before booking: placenta previa, preeclampsia, a high-risk pregnancy for any reason, blood clots or DVT, or any condition your OB has told you requires restricted activity. For these situations, massage may still be possible but needs explicit medical sign-off.

For healthy pregnancies in the second and third trimesters, prenatal massage has a strong safety record and significant documented benefits.

What Prenatal Massage Addresses

Lower back pain: One of the most universal complaints of pregnancy. As the belly grows, the lumbar curve increases, and the muscles of the lower back and sacrum take on extra load. The ligaments in the pelvis, loosened by the hormone relaxin, can also become a source of pain — especially the SI joint area (sacroiliac, at the dimples of the lower back). Prenatal massage that targets the lower back and gluteal muscles provides direct, meaningful relief here. Many women describe their lower back session as the first time in weeks their back doesn't hurt.

Hip pain and sciatic nerve pain: As the uterus grows, it can put pressure on the sciatic nerve, causing pain that runs from the lower back down through the buttock and into the leg. The piriformis muscle — a small but significant muscle deep in the buttock — often becomes tight during pregnancy and directly compresses the sciatic nerve. Careful, targeted work on the piriformis and surrounding hip musculature is one of the most effective treatments for pregnancy-related sciatic pain.

Swollen feet and ankles: Edema — fluid retention — in the feet and ankles is extremely common during pregnancy, especially in the third trimester and in hot weather. It also helps with the general heaviness and achiness that swollen feet cause.

How Prenatal Massage Differs from Regular Massage

The most obvious difference is positioning. In the second and third trimesters, lying flat on the stomach is not possible, and lying flat on the back for extended periods is not recommended (it compresses the vena cava and can reduce blood flow to the baby).
